The Mirror Lies. The Scale Lies. Your Bloodwork Does Not.
You can look great on stage. You can hit PRs in the gym. You can post your physique and feel like the work is paying off. None of that tells you what is happening inside the body that is producing those results. Bloodwork does. It is the one place the body cannot pose.
For an enhanced athlete, that distinction is not optional. It is the entire game. A protocol that is producing the physique you want while quietly destroying your lipids, organs, or hormones is not a winning protocol. It is a debt you have not been billed for yet.
Most coaches ignore bloodwork because they do not know how to read it. Most doctors flag everything outside "normal" because they were never trained on enhanced athletes. Both leave you without the information you actually need to make a decision.
Why "Normal" Is The Wrong Word
Standard reference ranges are built off a population of people who are largely sedentary, untrained, and dealing with their own underlying issues. Comparing your numbers to that population tells you whether you are sicker than the average sick person. It does not tell you whether you are operating at the level your physique and performance demand.
What your lab will print on the report. The range your doctor uses to flag you as "fine" or "concerning." Built off general population data, not enhanced athletes.
What you actually want to see if you are training hard, building, and operating at a high level. This is the range health-first enhanced coaching works toward, not just the range that keeps you out of the danger zone.
How To Use This Blueprint
- Get a comprehensive panel pulled. Not just "Total T and a CBC." A full panel with hormones, lipids, metabolic, organ function, and CBC. This document tells you which markers matter.
- Match each result against both ranges. Standard tells you if a doctor will flag it. Optimal tells you if it is sustainable for an enhanced athlete.
- Read the enhanced-athlete context next to each marker. That is the part your doctor will not give you.
- Use the Red Flag Checklist to triage what needs immediate action vs. what to monitor over time.
- Track quarterly using the template at the back. Trends tell the truth. Snapshots can lie.
5 Categories · 22 Markers · The Complete Picture
If you are only looking at hormones, you are looking at one fifth of the picture. The body is a system. A high estradiol does not exist in isolation — it pulls on cardiovascular markers, blood viscosity, mood, and recovery. To make a real decision about your protocol, you need to see all five categories at once.
Total T, Free T, Estradiol, SHBG, Prolactin. The direct readout of what your protocol is actually doing systemically.
Total Cholesterol, HDL, LDL, Triglycerides, ApoB. The first system to take damage. Often the last system you check.
Fasting Glucose, Fasting Insulin, HbA1c, hsCRP. Tells you if your body is still using fuel cleanly or moving toward dysfunction.
ALT, AST, GGT, Creatinine, eGFR. Liver and kidney are the workhorses. They tell you what your protocol is costing them.
Hematocrit, Hemoglobin, RBC, Ferritin. Erythrocytosis is the most under-monitored issue in this space. Read this one carefully.
Looking at one category in isolation is like checking your speedometer with the engine on fire. You only get the truth when all five categories are read against each other.
22 Critical Markers · Standard Range · Optimal Range · Enhanced Context
Hormones & Androgens
This is the category most enhanced athletes obsess over and most doctors mishandle. The numbers here are downstream of your protocol. They tell you what is actually circulating, not what you wrote on the protocol planner.
Trough/peak timing matters. A 1,800 ng/dL pull 2 days after a pin reads very different than 1,800 baseline. Without context, the number alone is meaningless.
Suggests dosing, ester timing, or absorption issue. On a structured protocol, a low TT means the protocol is not delivering, not that you need to "push more."
Total T tells you almost nothing on its own. It must be read alongside SHBG and Free T. Two athletes with the same TT can have wildly different real androgenic activity depending on SHBG. Always pull all three together.
This is the marker that actually drives androgenic activity. Very high Free T with normal Total T usually means low SHBG, which has its own implications.
Even with "normal" Total T, low Free T can mean SHBG is binding too much, or your protocol is not producing the active fraction you assumed.
Free T is the number that actually correlates to physique, libido, and recovery. If you are only paying attention to Total T, you are missing the marker that matters.
Water retention, soft look, mood swings, gyno symptoms. But crashing E2 with an AI is almost always worse than running it slightly elevated. Read with care.
Joint pain, low libido, depressed mood, poor pumps, lipids worsen. Crashed E2 is one of the most common, most painful self-inflicted issues in this space.
Always run the Sensitive E2 assay (LC-MS/MS), not the standard immunoassay. The standard assay can read 2 to 3x the real value in males and lead to wildly incorrect AI dosing. Insist on the sensitive version.
Binds more T, lowering Free T. Symptoms can mirror low T even with high TT. Often pulled up by certain compounds and oral hormonal influences.
More Free T circulating, but very low SHBG (under 10) is its own red flag, often correlated with insulin resistance, thyroid issues, or certain compounds.
SHBG is one of the most underrated markers in enhanced athlete coaching. It changes how every other androgen reading should be interpreted. Without it, your hormone panel is incomplete.
Low libido, ED, milk-like discharge, depressed mood, recovery issues. Often elevated by certain 19-nor compounds and sustained high E2.
Less commonly an issue, but very low values can correlate with dopaminergic excess from certain compounds. Usually not flagged unless symptomatic.
If you are running 19-nor compounds and not pulling Prolactin quarterly, you are flying blind. This is one of the cheapest, easiest markers to track and one of the most diagnostic for protocol-related symptoms.
Cardiovascular Markers
This is the category that decides whether you are around in 20 years to enjoy the physique you are building. It is also the category enhanced athletes most often ignore, because the damage is silent until it is not. You do not feel cardiovascular damage building. You feel it the day it stops being damage and becomes an event.
Less informative on its own than people think. Elevated TC with high HDL and low ApoB is very different from elevated TC with crushed HDL.
Very low TC (under 130) can indicate issues with hormone production substrate, since cholesterol is a precursor to most steroid hormones.
Total Cholesterol alone is a weak signal. Always read with HDL, LDL, Trigs, and ideally ApoB. The ratios tell the story, not the headline number.
Generally protective. Very high (over 100) on certain compounds is rare but worth context.
Major red flag for enhanced athletes. Many compounds, especially orals, crush HDL. A doctor saying "eh, your HDL is a bit low, not a big deal" does not understand the protocol context.
HDL is one of the first markers to take damage on harsh protocols. If your HDL drops 40% on a cycle, that is your protocol talking. Take this one seriously, even when your doctor does not.
Cardiovascular risk marker. Many compounds push LDL up. This is one of the levers your protocol design pulls on, intentionally or not.
Generally favorable. Very low LDL alongside very low HDL is less reassuring than it looks.
LDL is one piece. ApoB is a stronger single signal for cardiovascular risk and is becoming the gold standard. If you can pull both, do.
Often a function of carbs, alcohol, fasting compliance, or insulin resistance, not just the protocol. Make sure your sample was a true 12 hour fast.
Generally favorable. Read alongside HDL.
A high Trig with a low HDL is a metabolic flag, regardless of compounds. The Trig:HDL ratio is one of the cleaner cardiometabolic risk signals available on a basic panel.
Strong predictor of atherosclerotic risk. Counts every atherogenic particle, including LDL and VLDL, in one number.
Favorable for cardiovascular risk. Match against the rest of the lipid panel for full context.
If your basic panel is missing ApoB, request it on the next pull. For an enhanced athlete, this single marker may be the most important cardiovascular number you can track.
Metabolic Health
This category tells you whether your body is still using fuel cleanly or whether years of high calories, training stress, and certain compounds have started to push you toward insulin resistance. Catch this early and the fix is straightforward. Catch it late and it is a much bigger problem.
Even within "normal." A fasting glucose creeping into the 95–99 range is an early signal. Some compounds, especially GH and certain orals, drive glucose up.
Usually a non-issue unless symptomatic. Very low fasting glucose can be a sign of overtraining or under-eating.
Glucose is a snapshot. Pair with HbA1c (the trend) and Fasting Insulin (the pressure on the system) for a real read.
The body is working harder to manage blood sugar than it should. This is the earliest signal of developing insulin resistance, often visible long before glucose moves.
Generally favorable. Very low insulin alongside very low fasting glucose can correlate with overtraining.
Most doctors will not pull insulin if glucose looks fine. Insist on it. A glucose of 90 with an insulin of 18 is a very different body than a glucose of 90 with an insulin of 4.
Reflects the average glucose level over the past 90 days. Movement from 5.2 to 5.6 is a meaningful trend, even though both are technically "normal."
Generally favorable. Read with Fasting Insulin and Glucose.
A1c is the trend. Glucose is the snapshot. Insulin is the workload. All three together tell you if metabolism is still operating cleanly.
Systemic inflammation. Could be from training stress, poor sleep, infection, gut issues, or protocol load. A persistent reading over 1.5 deserves investigation.
Generally favorable. Indicates a low systemic inflammatory load.
hsCRP is the closest thing to an "overall stress on the system" marker available cheaply. If your hsCRP is climbing alongside your training load and protocol, the system is telling you to back off something.
Organ Function
The liver and kidneys are the workhorses of any enhanced protocol. They process compounds, manage clearance, and absorb a disproportionate amount of the cost. They will tell you when they are tired, but only if you are pulling the right markers.
Liver enzyme elevation. Can come from intense training in the 48 hours before the draw, certain orals, or actual hepatic stress. Context matters heavily.
Generally not concerning unless extremely low.
Do not train heavy in the 48 hours before bloodwork or you will get a false-elevated ALT/AST. If your enzymes are still high after a clean draw, that is a real signal, not a training artifact.
Same training caveat as ALT. AST is also produced by muscle, so post-training elevation is common and not always hepatic.
Generally not flagged.
Read ALT and AST as a pair. A persistently elevated ALT with a relatively normal AST points more toward the liver than muscle. Both elevated together points more toward post-training noise.
More liver-specific than ALT/AST. Less impacted by training. Often the cleanest signal of actual hepatic stress, alcohol load, or oral compound impact.
Generally favorable.
If you want a single liver marker that cuts through the training noise, GGT is the one. Always include it on enhanced athlete panels.
For lean, muscular individuals, slightly elevated creatinine is often a function of muscle mass, not kidney damage. eGFR is muscle-mass-biased and can read artificially low.
Generally not concerning. Very low values can be a sign of low muscle mass.
A 220-pound lean enhanced athlete will almost always have higher creatinine than the reference range was built for. If your eGFR reads "concerning," request a Cystatin C-based eGFR for a more accurate read.
Blood Health
This is the most under-monitored category in the entire enhanced athlete space. Hematocrit creep is the issue that takes athletes out, often before they ever see it coming. Read this section twice. If you are running protocols and not pulling a CBC quarterly, you are gambling.
Thicker blood. Higher cardiovascular risk, higher clotting risk, higher blood pressure. Above 52% is a real red flag, above 54% is urgent.
Less common in enhanced athletes. Can indicate iron deficiency, blood loss, or chronic inflammation.
HCT will quietly creep up over months. Donating blood, reducing protocol load, addressing hydration, and tracking quarterly are the levers. Do not let HCT cross 52% before you act.
Tracks alongside HCT. High HGB is the same conversation as high HCT, with the same management levers.
Anemia signal. Combined with low ferritin or low iron, points to iron deficiency. Often missed in heavy-training athletes.
Read with HCT and RBC. The three move together. If only one is moving and the others are not, the signal needs investigation.
Often correlates with elevated HCT and HGB. Same conversation, same management.
Investigate alongside HGB and ferritin for anemia, blood loss, or chronic inflammation.
RBC, HGB, and HCT are a trio. Always interpret together. The pattern of how they move tells you more than any single marker.
Iron storage is high. Can be elevated by inflammation (acute phase reactant) or actual iron overload. Read with hsCRP for context.
Iron stores are running down. Can drop fast in athletes who donate blood frequently to manage HCT. Watch for fatigue, drop in performance, paler skin.
Athletes who donate blood quarterly to manage HCT will eventually deplete ferritin. Track this so you know when iron supplementation or a donation pause is needed. Do not just keep donating without checking ferritin.
Triage: What Actually Needs Action
Not every out-of-range number is an emergency. The job of this checklist is to take the anxiety out of bloodwork review while keeping you health-focused. Some markers demand action this week. Some need a 90-day plan. Some are just data points to track. Knowing which is which is the entire skill.
- Hematocrit over 54% or rising rapidly. Donate blood, reduce protocol load, recheck within 30 days.
- ApoB over 130 mg/dL while on protocol. Cardiovascular risk needs immediate attention.
- HDL crashed below 25 mg/dL. Many compounds do this. Once you see it, the protocol needs to change.
- ALT/AST over 3x the upper limit on a clean draw (no training 48 hours prior).
- Fasting glucose over 110 mg/dL with HbA1c over 5.9%. Pre-diabetic territory, do not wait.
- Persistently elevated Prolactin with symptoms (low libido, ED, mood changes) on 19-nor protocols.
- HCT 50–54%. Not urgent yet, but trending. Plan a donation, hydration audit, and recheck.
- ApoB 90–130 mg/dL. Adjust protocol levers, address diet, recheck in 90 days.
- HDL 25–40 mg/dL. Address through compound selection, fish oil, training, fiber, recheck.
- Fasting insulin 8–15 mIU/L with normal glucose. The stress is starting to show. Address now, not later.
- hsCRP persistently 1.5–3.0 mg/L. Audit training load, sleep, gut, protocol stress.
- Crashed E2 (under 12 pg/mL on sensitive) with joint pain, low libido, low mood. Reduce or remove AI.
- HCT 48–50%. Watch the trajectory across two pulls. Trend matters more than the number.
- SHBG outside 20–45 nmol/L without symptoms. Track but do not panic-correct.
- Single elevated ALT/AST on a draw within 48 hours of training. Re-pull on a clean draw before reacting.
- Slightly elevated creatinine in a lean, muscular athlete. Confirm with Cystatin C eGFR before acting.
- Total T outside expected range in a way that fits your protocol. Confirm timing of pin and draw before adjusting.
Tier 1 is for the 5% of pulls that need action this week. Most of what you will see lives in Tier 2 or Tier 3. Knowing the difference is what separates an enhanced athlete who lasts decades from one who flames out in five years.
Connect The Dots Between What You Are Running And What You Are Seeing
Most athletes look at bloodwork results in a vacuum. They see a low HDL and panic, or a high HCT and shrug. The real skill is connecting the markers back to the protocol. Different compounds and protocol structures pull on different markers in predictable ways. Once you see the pattern, the bloodwork becomes a feedback loop, not a mystery.

If you can read a panel and immediately know which lever pulled which marker, you stop guessing. That is the entire point of this blueprint.
Trends Tell The Truth · Snapshots Can Lie
One bloodwork pull is a snapshot. Four bloodwork pulls a year is a trend. The trend is what tells you whether your protocol is sustainable, whether you are creeping toward a problem, or whether you have already crossed a line you did not see coming. Use this template to track your key markers across four quarterly pulls.

How To Read Your Trend
- Look at direction first, not absolute value. A marker moving from 90 to 95 to 100 over three pulls is more important than the single number.
- Group the categories. If 3 of your 5 lipid markers are worsening together, that is a category-level signal, not a coincidence.
- Tie movement to protocol changes. If you started a new compound at Q2 and 3 markers shifted at Q3, the math is not subtle. Connect it.
- Pull at the same time of day, in the same fasted state, with no training in the 48 hours prior. Eliminate noise to see the real signal.
- Bring this sheet to your coach or physician. A trend across 4 pulls is far more useful in a conversation than one isolated panel.
Common Mistakes Reading Bloodwork
- Reacting to one bad pull. Re-test in 30 days before changing your protocol off a single number.
- Ignoring the trend because the absolute value is "in range." Movement from 5.2 to 5.6 HbA1c is a real story, even though both are technically normal.
- Crashing E2 with an AI off a standard (non-sensitive) reading. The standard assay is unreliable in males. Run sensitive every time.
- Pulling enzymes 12 hours after a leg session. You will get a false-elevated ALT/AST and panic. Wait 48 hours.
- Donating blood every month forever. Address the underlying protocol load instead of just managing the symptom.
